What should I bring to my initial appointment?

To save time, this form can be downloaded here and returned prior to your visit, or you can complete it when you come in.

  • Referral letter

  • All relevant test results including blood tests, urine tests and imaging (e.g. MRI/CT/Ultrasound)

To provide the best care, it is important that Dr Reeves can review all images in addition to reading the radiologist report. Please ensure that you check with the practice team that we can access your images online. Otherwise, you may need to bring a copy of your scans on CD.
